Participating insurance network list

We have contracted with many insurers and health plans to accept an assignment of benefits. This means we will bill the plans with which we have agreements, and you will only need to pay the authorized co-payment, deductible, and/or co-insurance at the time of service.

CMDP

The Comprehensive Medical and Dental Program (CMDP) is a healthcare plan in Arizona that provides medical, dental, and behavioral health services specifically for children in foster care. As of April 1, 2021, CMDP transitioned to the Mercy Care Department of Child Safety Comprehensive Health Plan (Mercy Care DCS CHP).

CareFirst

As an independent licensee of the Blue Cross Blue Shield Association, CareFirst is a health insurance provider that offers a wide range of medical, dental, and vision plans. It serves individuals and families in Maryland, Washington, D.C., and Northern Virginia.

IHS

The Indian Health Service (IHS) is a federal agency within the Department of Health and Human Services responsible for providing healthcare services to American Indians and Alaska Natives.

APIPA – CRS

APIPA-CRS, formerly known as the Arizona Physicians IPA (APIPA), is now part of UnitedHealthcare Community Plan. The CRS component stands for Children's Rehabilitative Services, which provides specialized medical treatment and services to children with qualifying medical conditions under the Arizona Health Care Cost Containment System (AHCCCS).

Health Choice

Health Choice Arizona, now known as BCBSAZ Health Choice, is a comprehensive health insurance plan serving members of the Arizona Health Care Cost Containment System (AHCCCS), Arizona's Medicaid program.

APIPA

APIPA, which stands for Arizona Physicians IPA, is a former health insurance provider that is now part of UnitedHealthcare Community Plan. It primarily served members of the Arizona Health Care Cost Containment System (AHCCCS), the state's Medicaid program.

Mail Handlers

The Mail Handlers Benefit Plan (MHBP) is a health insurance program sponsored by the National Postal Mail Handlers Union (NPMHU), which is a division of the Laborers' International Union of North America (LIUNA). This plan has been serving federal and postal employees and retirees for over 60 years, offering comprehensive health coverage through the Federal Employees Health Benefits (FEHB) Program.

Golden Rule

Golden Rule Insurance Company, a part of the UnitedHealthcare family, specializes in individual and family health insurance products, including short-term medical insurance.

Great West

Great-West Healthcare, previously an independent health insurance provider, was acquired by Cigna in 2007. Following the acquisition, Great-West Healthcare's offerings were rebranded under the Cigna name.

First Health Plan

First Health Plan, under First Health Group Corporation offers one of the largest Preferred Provider Organization (PPO) networks in the United States, known as the First Health Network, along with several regional networks like Cofinity and First Choice of the Midwest.

HealthNet

Health Net, LLC, a subsidiary of Centene Corporation, provides a range of health insurance plans designed to meet the needs of individuals, families, businesses, and those eligible for government programs like Medi-Cal and Medicare.

GEHA

Founded in 1937 as the Railway Mail Hospital Association, GEHA (Government Employees Health Association) is a non-profit health insurance provider offering medical and dental plans specifically for federal employees, retirees, and their families.

Cigna

Cigna is a global health service company that offers a wide range of health insurance plans and services. Founded in 1792, Cigna is headquartered in Bloomfield, Connecticut, and serves millions of customers worldwide.

Humana

Humana is one of the largest health insurance companies in the United States, offering a range of health insurance products primarily focused on Medicare plans and group health plans for employers.

Blue Cross/Blue Shield

Blue Cross Blue Shield (BCBS) is a federation of 34 independent and locally operated companies that provide health insurance coverage across all 50 states, Washington, D.C., and Puerto Rico.

Aetna

Now a subsidiary of CVS Health, Aetna is a prominent health insurance provider offering a wide range of plans and services to individuals, families, employers, and Medicare beneficiaries.
